Position Overview

We are seeking two compassionate and dedicated Registered Nurses (RNs) to join a home health care team in Terre Haute, Indiana. This role offers an opportunity to provide personalized, patient-centered nursing care to geriatric patients in their homes while supporting their health, comfort, and independence.

The ideal candidate will have strong clinical and communication skills, a commitment to quality patient care, and preferably previous experience in home health or hospice settings.

Key Responsibilities

• Provide skilled nursing services to patients and families in their homes.
• Conduct admission assessments and patient information visits as needed.
• Educate patients, family members, and caregivers about care plans, disease processes, medications, and related health concerns.
• Develop, implement, and revise individualized plans of care in collaboration with physicians and the interdisciplinary care team.
• Support patients and families through physical and emotional challenges.
• Maintain accurate and timely electronic and paper medical records.
• Monitor patient conditions and report significant changes to physicians and other healthcare team members.
• Update physician orders in accordance with established policies and procedures.
• Coordinate care and participate in case conferences and interdisciplinary team meetings.
• Administer medications and follow infection prevention and control protocols.
• Supervise and evaluate Home Health Aide activities.
• Participate in staff meetings, in-service training, orientation, and other professional development activities.
• Maintain patient confidentiality and comply with professional nursing standards and organizational policies.

Qualifications And Requirements

• Current and active Indiana Registered Nurse (RN) license.
• Minimum of one year of nursing experience.
• Bachelor of Science in Nursing (BSN) preferred.
• Previous home health or hospice experience preferred.
• Experience caring for patients with life-threatening illnesses is desirable.
• Knowledge of and commitment to the hospice philosophy of care.
• Understanding of pain management and symptom management principles.
• Excellent communication, interpersonal, and active listening skills.
• Valid driver's license and automobile insurance.
• Ability to complete required pre-employment screening, including a criminal background check, post-offer physical examination, and drug screening.

Schedule and Compensation

• Work Schedule:
  
  Monday-Friday, 8:00 AM-5:00 PM.
• Employment Type:
  
  Full-time.
• Caseload:
  Approximately 30-35 patients.
• On-Call Rotation:
  Every fifth month.
• Weekend Differential: $1.50.
• On-Call Differential: $5.00 Monday-Thursday and $6.00 Friday-Sunday.
• Salary:
  Based on years of experience and the applicable pay scale.
• No sign-on bonus or relocation assistance is offered.
