GP Practice Nurse - 22 hours per week initially, with view to increased hours in the future.

Based in a modern purpose built medical centre in the heart of Tewkesbury, opposite the community hospital we are looking for a registered nurse to join our dedicated nursing team.

A specialist interest/qualification in COPD and sexual health is a must for our team due to retirement of a long standing team member.

We are looking for someone who thrives as part of a team and enjoys working collaboratively with colleagues across all areas of the practice.

Working Pattern: (minimum hours required)

Tuesday  6 hrs

Friday 08.00 - 17.00 8hrs

Inclusion Enhanced Access rota - Monday evening(6.30pm to 8pm) and Saturday morning (8.30am to 12.30pm) (additional pay)

Main duties of the job

Key responsibilities

  • Deliver nurse-ledCOPD reviews, including assessment, monitoring and patient education/self-management support in practice and visiting housebound patients and care homes.
  • Administering vaccines and Partaking in vaccination clinics at the practice and also visiting housebound patients when required.
  • Providesexual health services, including contraception advice, reviews, initiation, contraceptive implant fitting and removal is desirable but not essential. STI screening (where locally commissioned), and health promotion
  • Support chronic disease management and preventative care in line with practice protocols, national and local guidelines.
  • Experience in wound care, ECGs, vaccinations and injections.
About us

Our 9 GPs (6 partners and 3 salaried) are supported by awider clinical team comprising of 2 nurse practitioners, 5 practice nurses, 3

HCAs and 2 part time clinical pharmacists. The administrative and management team is made up of long-standing staff with a varied and strong administrative and management wealth of experience and knowledge, including nursing specialisttrained in chronic disease management, QOF recalls management team and a teamof pharmacists and pharmacy technicians.

We are a training practice hosting up to 3 GP registrars atany one time and we are currently supporting 2 medical students from Three Counties Medical School.

Our internal focus is on wellbeing of our staff, achievedwith various protected learning events, staff wellbeing sessions (such as yogarun by one of our salaried GPs), regular meetings sharing clinical knowledge and providing support to colleagues.

Summary of key responsibilities:

Consult with patients and their families to undertake assessments of patient need to support them in managing long term conditions and improve patient health outcomes locally and nationally.

Consult with patients day to day and provide routine nursing care to patient.

Work with teams across the practice, primary care network and wider neighbourhood teams.

Contribute to safe working, ensuring practice protocols are followed and updated when required.

Person Specification Qualifications
  • Registered Nurse RN with current NMC registration
  • Experience in general practice nursing and long-term condition clinics
  • Demonstrable experience and specialist interest and qualification in COPD/Asthma.
  • Strong communication skills and ability to work autonomously within agreed clinical protocols
  • Commitment to high standards of patient care, safety, and confidentiality
  • Immunisations and cervical screening competencies
