The Role

The Clinical Stabilization Services Program (CSS) includes education and counseling regarding addiction, relapse prevention, and aftercare. The Case Manager assists clients during the post-detox process to identify goals, needs, and resources during this time-limited stay. Together with the client, the Case Manager formulates a plan to meet those goals, ranging from returning home to further residential placement.

What You'll Do
  • Greet new clients and create a welcoming, comfortable atmosphere.
  • Interview clients, review records, and confers with other professionals to evaluate condition.
  • Obtain and document in a precise and timely manner all relevant intake material, assess continuing care needs, contact appropriate aftercare resources, and arrange transfer or admission to appropriate levels of care, or liaise with self‑help organizations.
  • Assist in formulating a treatment plan based on a comprehensive biopsychosocial assessment of the client.
  • Share responsibility for educational and other group programming.
  • Counsel clients individually, in family and group sessions to assist in achieving treatment goals under the direction of the Clinical Supervisor.
  • Refer clients to other support services as needed such as medical evaluation, social services, and employment services.
  • Prepare and maintain reports and case histories per agency guidelines for format, timeliness, and quality assurance.
  • Supervise toxicology screens.
  • Search clients' belongings upon admission to the program.
  • Enter client data into paper and electronic records.
  • Complete a psychosocial history, tobacco assessment, and wellness assessment within regulation deadlines.
  • Possess knowledge of relevant community resources for continuing care/aftercare placement.
  • Respond promptly to client needs; maintain confidentiality.
  • Complete discharge notes and client disenrollment forms to close cases upon client discharge.
  • Attend staff meetings and supervision sessions.
  • Perform all other related duties as assigned.
Qualifications
  • Associate degree with certifications, or a combination of experience and education; bachelor’s degree preferred.
  • Bilingual Spanish and English required.
  • Possess compassion, patience, empathy; ability to relate to a substance‑use treatment population in a sensitive manner.
  • Interest and willingness to work in a multicultural setting.
  • Customer service orientation; manage difficult or emotional client situations.
  • Focus on solving conflict, not blaming; maintain confidentiality.
  • Knowledge of relevant community resources for continuing care/aftercare placement.
Schedule

Tuesday to Saturday, 9 am‑5 pm or 10 am‑6 pm. Tewksbury Hospital, 365 Main Street, Tewksbury, MA.

Physical & Mental Requirements

While performing the duties of this job, the employee is regularly required to walk, stoop, kneel, crouch or crawl and talk or hear. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required include distance vision, peripheral vision, and depth perception.
