Position Summary

Registered Nurse (RN) – Home Care Initial Assessments / Nurse Visits / Field RN (Fitchburg, Leominster, Milford, Worcester, Framingham areas). Based at Bright Star Care  of Acton/Andover & Lowell (main office: Tewksbury).

Key Responsibilities
  • Perform comprehensive patient assessments in the home environment, including physical, psychosocial, and environmental assessments.
  • Develop and implement individualized care plans based on patient needs, physician orders, and best practices.
  • Provide direct nursing care, including wound care, medication administration, catheter care, injections, and other skilled nursing tasks.
  • Monitor patient condition and progress; communicate changes to the physician and care team.
  • Educate patients and caregivers on disease processes, medications, self‑care techniques, and health maintenance.
  • Accurately document all assessments, interventions, and communications in the EMR system in a timely manner.
  • Collaborate with multidisciplinary team members, including physicians, physical therapists, occupational therapists, and social workers.
  • Ensure compliance with all state and federal regulations and agency policies.
  • Participate in case conferences, staff meetings, and ongoing training as required.
Qualifications
  • Current RN license in MA.
  • Minimum 6 months to 1 year of nursing experience, preferably in home health, community health, or acute care settings with assessments.
  • CPR certification (BLS) required.
  • Strong clinical and assessment skills.
  • Excellent communication, time management, and organizational skills.
  • Valid driver’s license and reliable transportation.
  • Ability to work independently and travel to patient homes.
  • Experience with EMR systems preferred.
