Registered Nurse (RN) – Home Care Initial Assessments / Nurse Visits / Field RN (Fitchburg, Leominster, Milford, Worcester, Framingham areas). Based at Bright Star Care of Acton/Andover & Lowell (main office: Tewksbury).
Key Responsibilities- Perform comprehensive patient assessments in the home environment, including physical, psychosocial, and environmental assessments.
- Develop and implement individualized care plans based on patient needs, physician orders, and best practices.
- Provide direct nursing care, including wound care, medication administration, catheter care, injections, and other skilled nursing tasks.
- Monitor patient condition and progress; communicate changes to the physician and care team.
- Educate patients and caregivers on disease processes, medications, self‑care techniques, and health maintenance.
- Accurately document all assessments, interventions, and communications in the EMR system in a timely manner.
- Collaborate with multidisciplinary team members, including physicians, physical therapists, occupational therapists, and social workers.
- Ensure compliance with all state and federal regulations and agency policies.
- Participate in case conferences, staff meetings, and ongoing training as required.
- Current RN license in MA.
- Minimum 6 months to 1 year of nursing experience, preferably in home health, community health, or acute care settings with assessments.
- CPR certification (BLS) required.
- Strong clinical and assessment skills.
- Excellent communication, time management, and organizational skills.
- Valid driver’s license and reliable transportation.
- Ability to work independently and travel to patient homes.
- Experience with EMR systems preferred.
