Job Opening:
Nurse Practitioner – Palliative Care

We are looking for a dedicated Nurse Practitioner (NP) to join our palliative care team at Ascend Palliative Care. The NP will conduct patient visits, collaborate with an interdisciplinary team, and contribute to quality palliative care.

• Conduct visits to patients on palliative care in accordance with local, state, and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
• Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, ongoing aggressive treatment).
• Consult with other members of the interdisciplinary team to contribute to the patient’s plan of care.
• Prepare and maintain accurate patient records, charts, and documents.
• Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors, and members of IDT.

• Master’s Degree in Nursing.
• Nurse Practitioner license in New Jersey.
• 2–4 years of experience working within hospice or a related field.
• Palliative care experience preferred.
• Valid driver’s license and reliable transportation.
• Willingness to travel.
