Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Dierksen Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.
Schedule: Monday - Friday, 8 am - 4:30 pm
Territory: Texarkana

- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
- Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services.
- Provide direct care, comfort, and support for patients, ensuring their comfort and quality of life remain paramount.
- Deliver top‑notch nursing care to ease pain and bring solace.
- Educate and advocate, empowering patients and families to create personalized care plans.
- Share call duties, ensuring presence when it matters most.
- Current unencumbered RN license.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice experience a plus.
- Computer proficient, typing and various programs, including EMR background.
- Current CPR certification if required by state.
- Current driver’s license, vehicle insurance, and dependable transportation.
$72,000 - $77,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
