• Provide direct patient care to residents, including vital signs, wound care, and administration of medications
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Conduct initial and ongoing assessments of residents' physical and emotional needs
• Develop and implement care plans that promote resident independence, dignity, and well-being
• Communicate effectively with residents, families, and healthcare team members
• Maintain accurate and detailed records of resident care and progress
• Participate in quality improvement initiatives to enhance resident outcomes and experiences

• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of Texas
