LPN – Acute Rehabilitation | Per Diem

Pay Rate: $33–$34 per hour
Schedule:
Per Diem | Flexible availability required
Shifts:
Days and Nights
Day Shift: 6:30 AM–6:00 PM
Position Overview
We are seeking a Licensed Practical Nurse (LPN) to provide high-quality patient care in an acute rehabilitation setting. This per diem opportunity is ideal for an LPN who can offer flexible availability for day and/or night shifts.
Key Responsibilities
- Provide direct nursing care to rehabilitation patients
- Monitor patients’ conditions and promptly report changes
- Administer medications and treatments as ordered
- Maintain complete and accurate clinical documentation
- Collaborate with RNs, physicians, therapists, and the interdisciplinary care team
- Support patient safety, comfort, and recovery
Requirements
- Active LPN license
- Acute care, rehabilitation, or similar clinical experience preferred
- Strong communication and patient-care skills
- Flexible availability for per diem shifts
- Ability to work day and/or night shifts
