We are hiring a Physician Assistant (PA-C) or Family Nurse Practitioner (FNP-C) to join our team.
What You’ll Do:- Provide thorough, personalized primary care to FPC members
- Build meaningful, long‑term relationships with a limited patient panel
- Communicate directly with patients by phone, text, and email
- Diagnose and manage chronic conditions with an emphasis on prevention and long‑term health
- Support patients with nutrition, exercise, sleep, stress management, and other lifestyle goals
- Perform procedures and expand your clinical skills within your scope of practice
- Collaborate with a supportive clinical team to continuously improve patient care
- Active
PA-C or FNP-C license - Fluent Spanish, spoken and written, required
- Strong communication and interpersonal skills
- Compassionate, professional approach to patient care
- Ability to handle sensitive information with confidentiality
- Strong follow‑through and care‑planning skills
- Passion for preventative and lifestyle‑focused medicine
