  • Please indicate the days of the week you are available.

  • Are you presently employed by another school district as a substitute (Y/N)? If so, please list all districts.
  • Are you currently making an active search for employment (Y/N)?
  • Would you accept any job assignment subject to qualifications (Y/N)? If no, please list exceptions.
  • Are you currently receiving retirement benefits from Texas Teacher Retirement System (Y/N)? If so, did you retire prior to January 1, 2001?
