The State Hospital Step Down program provides care in a Transitional Living Home with the 24/7 supports of the Intensive Transition Team (ITT), which the Registered Nurse will serve as a member of. This growing program is designed to step down/transition individuals who are psychiatrically and/or medically fragile from inpatient state hospital settings to more appropriate community-based settings. The ITT will work as a team to identify, assess, facilitate step down/transition, house and provide services and support to maintain community integration.
