Job Overview

Southwest Health and Human Services currently has an opening for a Long Term Care (LTC ) Social Worker. This position’s preferred locations are Slayton or Ivanhoe.

Southwest Health and Human Services (SWHHS) is a multi-county agency committed to strengthening individuals, families and communities by providing quality services in a respectful, caring and cost-effective manner.

This position’s purpose is to provide assessment and case management services in the elderly waiver and alternative care programs. It also includes care coordination for Minnesota Senior Health Options and Minnesota Senior Care Plus programs for community well, and elderly waiver.

• A bachelor’s degree from an accredited four-year college or university with a major in social work, psychology, sociology, or closely related field.
• OR a bachelor’s degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency.

Starting Rate of Pay: $26.62 – $44.18 hourly
