Physical Therapist - PRN

We have an amazing opportunity for a Per Diem (PRN) Physical Therapist. We are looking for a PT to join our in‑house team of outstanding professionals that service our residents through person‑centered care to achieve positive outcomes.

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensuring all treatment delivered is in accordance with the established plan of care.
• Monitoring patient response to treatment intervention and modifying treatment approach and directive as clinically appropriate with IDT collaboration.
• Providing clinical support and education to patients, family members, and caregivers.
• Collaborating with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensuring documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attending required meetings as designated by the Director of Rehab.

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

• Active/Valid Texas license in Physical Therapy.
• In good standing with all regulatory agencies and licensing boards.

• In‑house Rehab (non‑contracted).
