• RN license in Illinois or equivalent qualifications in human services
• Experience working with diverse populations, including racial and ethnic minorities and individuals with specific health needs
• Strong understanding of long-term care services and resources
• Excellent communication and interpersonal skills

• Experience in case management or a related healthcare role
• Familiarity with community resources and support services for long-term care
• Knowledge of state and federal healthcare regulations
• Additional certifications in relevant fields, such as social work or nursing
• Experience with holistic care management approaches
