Physical Therapist

We are seeking a qualified Physical Therapist to join our dynamic team at Avir at Hearne in Hearne, TX. As a PRN Physical Therapist with Avir at Hearne, you will be responsible for providing high-quality physical therapy services to patients, including evaluating and treating patients, developing treatment plans, and educating patients and families on self-care and wellness strategies. You will work collaboratively with a multidisciplinary team of healthcare professionals to ensure comprehensive care is delivered to our patients.

• Assess patients with physical therapy needs, developing and implementing individualized treatment plans to achieve optimal outcomes
• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
• Maintain accurate records of patient treatment and progress
• Participate in patient education
