Job Description
Provides professional skilled nursing care directly and indirectly to patients, primarily delivered on a visiting and intermittent basis in the patient’s home. Supervises the care provided by other department staff members. OSHA Bloodborne Pathogens Class I.
Responsibilities- Provides skilled nursing care to patients in their homes.
- Works in patient homes and occasionally in the office.
- Is on a rotating on‑call schedule to assist patients, family members, and staff with after‑hours patient care needs.
- Initiates plans of treatment, including preventative and rehabilitative nursing procedures.
- Assesses and interprets data on neonate and the pediatric, adolescent, and geriatric patients’ status to identify patients’ requirements relative to their age‑specific needs and provides care as stipulated in departmental policy.
- Maintains records of home visits, progress notes, and submits charge information for patient care.
- Informs physicians and hospice staff members of current and changing patient conditions.
- Provides education and training to patients, family members and staff members.
- Supervises the work and patient care of LVNs and aides.
- Maintains confidentiality while working with confidential matters on a daily basis.
- Maintains continuous relationships with physicians, patients, and family members.
- Follows aseptic procedures and precautions when involved in direct patient contact or handling potentially infectious materials.
- Attends educational in‑services and staff meetings as required.
- Follows safety guidelines and policies for the hospital district.
- Education:
Graduate of an accredited School of Professional Nursing. - Job related skills:
Effective verbal and written communication skills; current knowledge of nursing principles, standards, and practices; ability to independently manage multiple priorities. - Licensure, Registry or Certification:
Registered Nurse currently licensed by the state of Texas and a current driver’s license. - Experience:
One year experience as a Registered Nurse; experience with Medicare and Medicaid preferred; current CPR certification. - Physical and mental requirements:
Moderate periods of sitting, standing and walking; occasional bending, stooping, reaching, pulling and pushing; lifting up to 50 pounds; occasional heavy lifting when supporting or moving patients who may become weak or helpless.
$34.00/hr - $42.00/hr
