Home Health nurses will be responsible for providing at-home nursing care for patients, often as follow-up care after discharge from a hospital, rehabilitation, long-term care, or skilled nursing facility. Key qualifications include clinical knowledge, interpersonal communication, compassionate care, and time management skills.

Home Health Nurse Responsibilities:

Assess patients' condition during visits and chart observations.

Perform evaluations, including vital signs, medication review, Oasis assessment, wound care, and IV therapy.

Dress or redress open wounds and assess healing progress.

Collect data and prioritize problems and needs.

Supervise CNA and LVN staff under your caseload.

Document according to company and compliance standards.

Educate patients and families on home health care strategies and procedures.

Coordinate with occupational therapists, physical therapists, physicians, and other care team members.

Recommend devices or tools to improve patients' quality of life.

Listen to patients and respond to concerns or requests.

Position available outside of 410 NW 78253 - Alamo Ranch and Helotes area.
