• Deliver quality patient care under RN supervision, following care plans and physician orders.
• Continuously observe and assess patient conditions, reporting changes to supervisor or physician.
• Document all activities, assessments, nursing actions, and coordination of care in a timely manner.
• Participate in nursing care plan implementation and updates.
• Implement interventions to prevent or reduce patient risk.
• Educate patients and families about disease processes, self-care, and prevention strategies.
• Maintain knowledge of nursing competencies through ongoing education.

• Active, unrestricted LPN/LVN license in the state of practice.
• Current CPR certification.
• TB skin test current within last 12 months.
• Reliable transportation.
• Six months prior hands‑on nursing experience preferred but not required.

• Recent experience as LPN/LVN in a clinical setting.
• Home health experience.
