Location: Dalhart

  • Conduct comprehensive assessments of patients' health status.
  • Develop, implement, and evaluate individualized care plans.
  • Administer medications, treatments, and other clinical procedures as needed.
  • Monitor and document patients' progress and response to treatment.
Case Management
  • Coordinate patient care activities and ensure continuity of care.
  • Facilitate communication between patients, families, and healthcare providers.
  • Advocate for patients' needs and assist in navigating healthcare systems.
  • Provide patient education on disease management, treatment options, and preventive care.
Clinical Coordination
  • Collaborate with physicians, specialists, and other healthcare team members.
  • Schedule and manage patient appointments, follow-ups, and referrals.
  • Ensure compliance with healthcare regulations and clinic policies.
  • Participate in quality improvement initiatives and clinical audits.
Administrative Duties
  • Maintain accurate and up-to-date medical records and case notes.
  • Prepare and submit reports on patient outcomes and case management activities.
  • Assist with the development and implementation of clinic protocols and procedures.
Job Qualifications
  • Education:
    • Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited institution.
    • Current and valid RN license, allowing practice in the State of Texas.
  • Experience:
    • Minimum of 2 years of clinical nursing experience, preferred.
    • Previous experience in case management or related field, preferred.
  • Skills:
    • Strong clinical assessment and critical thinking skills.
    • Excellent communication and interpersonal abilities.
    • Proficient in electronic medical records (EMR) and other healthcare software.
    • Ability to manage multiple priorities and work in a fast-paced environment.
    • Compassionate and patient-centered approach to care.
