Night Shift LPN

We are seeking a compassionate and skilled Night Shift LPN to join our dedicated team at Avir  a Night Shift LPN, you will play a vital role in providing exceptional care and support to our residents while contributing to a warm and welcoming environment.

As a Night Shift LPN, you will be responsible for:

• Providing direct care to residents, including administering medications, wound care, and vital sign monitoring

• Developing and implementing individualized care plans in collaboration with the healthcare team

• Assisting with resident transfers, repositioning, and ambulation

• Monitoring and reporting resident changes and concerns to the nursing staff

• Maintaining accurate and up-to-date records and documentation

• Provide exceptional care and support to residents, adhering to established standards and policies

• Collaborate with the healthcare team to develop and implement individualized care plans

• Participate in quality improvement initiatives and maintain a safe and clean environment

• Valid LPN license in the state of Texas

• Minimum 0 years of experience in long-term care or a related field

• Ability to work variable shifts, including nights and weekends

• Strong communication and interpersonal skills

• Ability to lift, transfer, and reposition residents with assistance
