As a Charge Nurse at Honey Grove Wellness & Rehabilitation, you will be responsible for providing high-quality patient care, taking charge of patient assessments, and developing and implementing individualized care plans. You will work closely with our interdisciplinary team to ensure seamless care transitions and communicate effectively with patients, families, and other healthcare professionals. Your LVN or RN Nurse skills will make a real difference in the lives of our patients, and we are eager to welcome you to our team!

• Provide direct patient care, including assessments, treatments, and medications as prescribed by physicians
• Develop and implement individualized care plans, taking into account patient preferences, needs, and goals
• Collaborate with interdisciplinary teams to ensure comprehensive care, including attending care conferences and contributing to case management
• Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care transitions
• Perform administrative tasks, including documenting patient information, charting, and reporting
• Participate in quality improvement initiatives and performance improvement activities to maintain high standards of patient care

• Current licensure as a Licensed Vocational Nurse or Registered Nurse in the state of Texas
