Key responsibilities include:

• Conducting initial assessments and developing individualized care plans to address patients' social, emotional, and spiritual needs
• Providing counseling and support to patients and their families, addressing concerns and improving coping skills
• Collaborating with interdisciplinary teams to ensure effective communication and coordination of care Identifying and addressing social determinants of health that impact patient outcomes
• Documenting patient information and progress accurately and timely

• Licensure as a Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)
• Strong communication, interpersonal, and organizational skills
• Ability to work effectively in a fast-paced, dynamic environment
• Familiarity with electronic medical records (EMRs) and ability to learn our system
