Independent Optometrist – Sam’s Club, 4691 State Highway 121, Colony, TX

Full-time.

As an Independent Optometrist leasing space inside our Sam’s Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well‑being.

• Launch your services in Sam’s Club.
• Perform eye examinations and provide vision therapies.
• Maintain autonomy over practice days, hours, patient schedule, and fee schedule.
• Keep 100% of exam and professional fees.
• May opt into fair‑market‑value equipment, administrative, and Opt‑Tech support as permitted by state.

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• General liability certificate.
• Professional liability insurance certificate.
• W‑9 form.
• For ophthalmology support:
  Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited school and a valid state ophthalmology license.

• License agreements are with Gumberg Asset Management Company (ME & VA).
