Admissions Coordinator (Full-Time and/or Part-Time)

Chatham Glen Healthcare and Rehabilitation Center is seeking an organized, compassionate, and detail-oriented Admissions Coordinator to join our team! This role is essential in welcoming new residents, supporting families through the admissions process, and ensuring a smooth transition into our facility. We are hiring for Full-Time and/or Part-Time positions.

What You’ll Do
  • Provide a warm, welcoming experience to potential residents and families during tours, calls, and inquiries.
  • Coordinate and manage the full admissions process—including assessments, payer verification, hospital day checks, referral tracking, and room assignments.
  • Work closely with Nursing, Social Services, Business Office, and Marketing to ensure timely and accurate admission information.
  • Complete admission packets, obtain necessary signatures, and maintain proper documentation within required time frames.
  • Verify Medicare/Medicaid, Managed Care, and insurance benefits, including co-pays and prior facility stays.
  • Ensure accurate census reporting, face sheet updates, and payer source documentation.
  • Serve as a liaison between staff, residents, families, and referral sources.
  • Assist with hospital visits when needed and support resident adjustment post-admission.
  • Participate in daily department meetings and prepare monthly admissions/trends reports.
What We’re Looking For
  • Experience in healthcare admissions, case management, or similar roles.
  • Strong customer service, interpersonal, and communication skills.
  • Ability to multitask, work independently, and maintain accuracy in a fast-paced environment.
  • Knowledge of Medicare, Medicaid, Managed Care, and insurance processes (preferred).
  • Proficiency with Microsoft Word, Excel, Outlook, and related software.
