Overview

Responsibilities
  • Practice medicine under the supervision of a physician using sound medical judgment and adhering to all ethical considerations of the practice of medicine; knowing limitations and seeking consult when advisable; keeping abreast of current technologies and industry standards.
  • Under the supervision of a physician, provide care for patients with acute and chronic illness encountered in the older adult patient population.
  • Under the supervision of a physician, be accountable for patient care and outcomes and appropriately seek consultation from specialists when needed; stay involved in the detailed care of patients.
  • Engage with hospitalists whenever a patient is in the hospital.
  • Meet all clinical obligations of the medical practice: accrediting agency compliance, CLIA compliance, drug prescription compliance, HIPAA, state and federal regulations, safety regulations, etc.
  • Assess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelines.
  • Follow all internal policies and procedures.
  • Accountable for the completion of all operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentation.
  • Maintain complete and detailed records electronically in the EMR of patients’ health care plans and prognosis, completed within 48 hours of each patient’s visit.
  • Provide patients with information needed to promote health, reduce risk factors, or prevent/manage disease or disability.
  • Analyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnosis.
  • Interpret data and diagnostic results for an appropriate course of action, including but not limited to blood gases, radiology, EKG, etc., in collaboration with supervising physician.
  • Exhibit behavior that emphasizes compassion, quality, loyalty, and conscientiousness.
  • Establish and maintain effective working relationships with patients, their families, and team members.
  • Document continuing education per licensure and certification requirements.
  • Ensure that patients are seen every other visit by the physician.
  • Maintain patient confidentiality in accordance with policy and procedure and HIPAA requirements.
  • Enact proper chain of command for patient complaints.
  • Perform additional duties as assigned.
Position Requirements / Skills
  • Graduate of an accredited school of nursing.
  • Certified nurse practitioner with current state licensure.
  • At least 1 year of professional nursing experience, including 1–3 years as a primary‑care nurse practitioner.
  • Prior experience working in an electronic medical records system.
  • Provide 5‑Star service in accordance with the Better Health Group’s mission.
  • Skilled in traditional nursing disciplines of patient assessment and management.
  • ACLS/BLS certification required as mandated by state.
  • Demonstrated knowledge of the rationale for appropriate patient care.
  • Ability to handle emergency situations calmly and effectively.
  • Ability to read, analyze, and interpret professional journals, technical procedures, and governmental regulations.
  • Excellent written and verbal communication and teaching ability; good telephone skills.
  • Working knowledge of Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint).
Physical Requirements
  • Full range of body motion, including handling and transferring patients and manual and finger dexterity; eye‑hand coordination.
  • Requires standing, walking, pushing, bending, kneeling, and reaching at arm’s length and overhead in a clinic for prolonged periods.
  • Must be able to lift and move patients and medical equipment as needed; up to 15 lbs.
  • Requires corrected vision and hearing to normal range.
  • Ability to sit for extended periods of time.
  • Ability to operate a motor vehicle and have own means of transportation.
