  • Evaluate the urgency of patient symptoms, proactively managing care and coordinating with members of the healthcare team via electronic communication or phone when cases require urgent attention beyond standard protocols.
  • Conduct telephonic assessments of oncology patients to determine the appropriate level of care, facilitating outpatient and telehealth visits when symptoms can be safely managed in those settings.
  • Educate patients on symptom management, medication side effects, and treatment plans, providing clear instructions and empathetic support to promote understanding, adherence, and self-management.
  • Collaborate closely with physicians, advanced practice providers, and other healthcare professionals to ensure comprehensive and coordinated patient care.
  • Document all patient interactions, assessments, interventions, and care elements thoroughly and accurately in the electronic health record (EHR) system.

  • Identify potential barriers to care and assist patients in navigating healthcare systems, resources, and support services to optimize access and continuity of care.
  • Accurately recognize high-risk conditions and elevate or direct patients to appropriate resources, including urgent or emergent care, when necessary.
  • Provide patient education and guidance that addresses physical, psychological, and spiritual aspects of care, ensuring a holistic and patient-centered approach.
  • Deliver professional nursing care under general supervision in accordance with national standards, organizational guidelines, and state licensing board scope of practice.
  • Participate in company-wide quality initiatives and support the oncology compliance program, including adherence to the Code of Ethics and Business Standards.
  • Engage in ongoing professional development, staying current with best practices in oncology nursing, telephonic triage, and telehealth care delivery.
  • Attend all required onsite meetings and trainings at the designated office location to maintain alignment with clinical protocols, technology updates, and organizational initiatives.
Essential Skills
  • Prior telehealth or remote triage experience providing clinical assessment and guidance over the phone or through virtual platforms.
  • Active and unrestricted Registered Nurse (RN) license from Texas or another compact state, with the ability to hold a Texas multistate or compact license as required.

  • Demonstrated proficiency in using electronic health record (EHR) systems for documentation and clinical workflows.
  • Strong critical thinking and problem-solving abilities to assess complex oncology symptoms and determine appropriate interventions.
  • Excellent verbal and written communication skills, with the ability to explain clinical information clearly and compassionately to patients and caregivers.
  • Ability to work independently and manage time effectively in a remote environment while maintaining high standards of clinical care and documentation.
  • Compassionate, patient-centered approach to care that recognizes physical, psychological, and spiritual needs.
