Physical Therapist (PT) – Long-Term Care
LP Therapy
LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.
Responsibilities- Review physician referrals and medical records
- Evaluate strength, mobility, and functional ability
- Develop and modify treatment plans
- Administer therapeutic exercises and modalities
- Educate patients and families
- Complete timely Medicare documentation
- Collaborate with care teams
- Supervise PTAs and aides as required
- Support quality outcomes and productivity standards
- Bachelor's degree in Physical Therapy
- Current state PT license
- LTC experience preferred
- Strong knowledge of Medicare guidelines
- Effective communication and teamwork skills
LTC facility; physical demands include lifting and mobility assistance.
