SUPERVISORDirector of Nursing
POSITION SUMMARYThe purpose of the LPN/LVN position is to participate in and oversee the provision of resident care as prescribed by the attending physician, consistent with accepted standards of care and within the scope of LPN/LVN training and licensure. LPN/LVN also provides direct resident care and assigns duties to subordinate nursing staff as appropriate.
DUTIES AND RESPONSIBILITIES- Ensures facility residents receive high-quality, individualized care.
- Administers medications and treatments as prescribed.
- Assess and reports resident changes in condition to the DON, Charge Nurse, attending physician, and responsible party as applicable. Documents change, notification of change, and new treatment orders/ plans. Explains treatment orders/ plans to residents and responsible party when needed.
- Receives, transcribes, and executes physicians' orders.
- Assists in collection of data relevant to resident treatment and assessments.
- Assures implementation of care plans as directed.
- Oversees/monitors tasks of subordinate staff.
- Evaluates staff performance and may initiate appropriate action.
- Answers phones and call lights.
- Keeps work area, medication carts, medication storage areas, and supply areas clean and organized.
- Provides and receives nursing report to maintain continuity of care.
- Investigates, documents, and initiates interventions as appropriate for resident incidents and accidents.
- May be required to investigate and document regarding staff or visitor incidents and accidents.
- Identifies, utilizes, and secures equipment and supplies.
- Delivers quality care consistent with accepted standards of care and established policy and procedure.
- Encourages safety to prevent accidents and assures equipment safety.
- Attends in-service training, Care Plan meetings, or any other meetings called by Legacy.
- Is responsible for using nursing judgement under the guidance of the Director of Nursing, RN Supervisor, Medical Director, attending physicians, nurse consultants, or other nursing professionals.
- Must act in ways that promote professional and positive representation of the facility to the community.
- Communicates in a professional and courteous manner using effective verbal and non-verbal communication skills with residents, their families, co-workers, and vendors.
- Maintains confidentiality of department, resident, employee information, communication, and documentation as well as compliance with the Health Insurance Portability and Accountability Act (HIPAA) regulations.
- Clocks in and out for all working/training hours. Working off the clock will not be permitted at any time or for any reason.
- Maintain compliance with Legacy's Compliance program and plan.
- Adhere to Legacy's Code of Conduct and Dress Code.
- These duties are not all-inclusive, but provide minimum performance expectations which will be reviewed in the evaluation process. Supervisor may edit, alter, add to, or exclude duties and responsibilities based on the needs of the facility.

Minimum Qualifications- Freedom from illegal use of drugs.
- Freedom from use and effects of drugs and alcohol in the workplace.
- Anyone found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care-related setting are ineligible for employment in the position.
- Graduation from an accredited nursing school and a current, valid LPN/LVN license per the employing state's requirements.
- Experience in Long-Term care setting preferred but not required.
- Current state license to practice as a nurse.
- Current CPR certification desired.
- Ability to read, analyze and interpret technical procedures or governmental regulations.
- Ability to write reports, nursing/Resident progress notes, and nursing procedures.
