Job Overview
  • Hours:

    Part‑time day, night, overnight shifts.
  • Location:

    Slayden House – Thomaston, GA.
  • Rate: $14.50 per hour.
  • Position type:
    Non‑exempt, full‑time or part‑time, hourly.
Scope of Role
  • Support a unit with 4–6 individuals in residential settings or up to 10 in periodic programs.
Key Responsibilities
  • Deliver services per individual plans: socialization, health maintenance, medication administration, skill/behavioral development.
  • Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
Essential Functions & Services
  • Direct support and assistance as per individual service plans.
  • Accurate record‑keeping, maintaining confidentiality.
  • Responsible management of individuals’ assets within budget.
  • Maintain healthy professional relationships with individuals, families, case managers.
  • Report abuse or neglect, medical and behavioral incidents per policy.
Health Care Responsibilities
  • Accompany to medical appointments; relay orders to providers.
  • Administer or support self‑administration of medication; document accurately.
  • Monitor health, communicate with nurse/supervisor, call 911 in emergencies.
  • Use medical equipment correctly; report malfunctions.
  • Prepare or assist with meals per menus; accommodate special diets.
Employment Responsibilities
  • Attend orientation, ongoing training, monthly staff meetings.
  • Work scheduled hours, notify supervisor in advance of absences, fill in for shifts as required.
  • Demonstrate teamwork, courtesy, respect.
  • Maintain a safe workplace, report hazards, participate in safety training.
Maintenance
  • Vehicle responsibilities: transport with safety, report accidents.
  • Housekeeping and environmental safety; perform minor repairs.
  • Ensure safety during bathing, participate in drills.
Other Duties
  • Perform additional duties as required.
  • Use Mobile Punch for timekeeping.
Minimum Knowledge & Skills
  • High school diploma or equivalent.
  • Six months of human services experience preferred.
  • Minimum 18 years of age.
  • Computer proficiency.
Certificates, Licenses, and Registrations
  • Valid driver’s license and vehicle insurance if providing transportation.
  • All state‑required training and certification completed.
  • Adherence to company MVR driving record requirements.
Work Environment
  • Work at program location; may accompany individuals into community.
Physical Requirements
  • Lift up to 100 pounds occasionally, 50 pounds frequently, 20 pounds constantly.
Addendum
  • CS New Jersey – Refer to CS NJ Addendum – Direct Support Professional _1001 _ MPA 3429.
  • Employees must cooperate with the licensee and Department of Human Services staff during inspections.
  • Must maintain valid driver’s license in good standing (state variations considered).
  • High school diploma or equivalent required.
  • Drug testing and criminal background checks as per policy.
  • Proficiency in all required training areas.
  • Minimum age 18 years.
