CLINICAL PHARMACIST - Pharmacy Oncology Drugs

DESCRIPTION

• Complies to the standards of pharmacy operation
• Assumes responsibilities for own professional education and assists in providing education to patients and other health care professionals
• Utilizes pharmacy operation standards as outlined in policies and procedures pursuant to patient specific services that affect patient outcomes, optimize drug therapy and minimize costs
• Utilizes effective communication and positive interpersonal skills
• Demonstrates competence on a consistent basis in regards to medication dispensing
• Performs duties in an independent manner and demonstrates desire to learn new tasks with little or no supervision
• Critically evaluates the patient’s drug regimen in context of the patient’s medical conditions, available laboratory data and other pertinent patient parameters
• Accurately detects, reports and monitors potential drug‑related problems
• Provides rational drug therapy recommendations based on information from the medical record, laboratory data, patient interview and other pertinent patient parameters
• Formulates an optimal pharmacotherapeutic plan which organizes and prioritizes interventions, lists options and provides follow‑up recommendations

Current GA Pharmacist license
