SPEECH-LANGUAGE PATHOLOGIST – THE VILLAS AT SUNNY ACRES - THORNTON, CO

Full‑time position at a skilled nursing facility providing long‑term care, post‑acute rehabilitation, and geriatric outpatient therapy. The role involves clinical development, career growth, and leadership training opportunities.

• Screen and evaluate patients with communication, cognitive, or swallowing disorders; develop plans of care per regulatory and clinical practice standards.
• Provide rehabilitative, skilled, and medically necessary treatment interventions for patients with communication, cognitive, and/or swallowing disorders.
• Evaluate treatment outcomes for effectiveness and modify services accordingly; make referrals based on outcomes.
• Offer consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
• Prepare comprehensive discharge summaries of services provided, in compliance with regulatory requirements.
• Document all regulatory and clinically required information in the electronic documentation system; maintain patient records per company policy.
