Dilico is seeking a committed Nurse Practitioner to join its dedicated Primary Care Team. You will collaborate with healthcare professionals to provide exceptional care to clients and their families. Candidates must hold a Master's degree in nursing, possess current registration as a Nurse Practitioner, and have at least two years of nursing experience, ideally in primary care or family medicine.

Key Responsibilities:

• Provide collaborative care with the Primary Care Team

• Utilize clinical decision-making to enhance patient outcomes

• Manage chronic diseases and implement preventative care

• Document patient interactions using electronic medical records

• Communicate effectively with patients and families

Requirements:

• Master's degree in Nursing, NP Extended Class

• Current Nurse Practitioner certification and licensure

• Minimum two years in nursing, preferably primary care

• CPR and Standard First Aid certification

• Valid Ontario Driver's License and access to a vehicle
