Position: Dynamic RPN Role
Help shape healthcare at Harbourview Family Health Team as a Registered Practical Nurse in Thunder Bay, Ontario. This full-time position focuses on delivering proactive, team-based patient care.
Harbourview Family Health Team is committed to high-quality care and is seeking a Registered Practical Nurse to join their growing team. In this role, you'll work alongside interdisciplinary professionals, supporting chronic disease management and patient assessments while ensuring compliance with CNO standards. Your work will contribute to our mission to enhance community health access and promote quality improvement initiatives.

Key Responsibilities:

• Conduct patient assessments and accurately document findings

• Provide routine nursing care, vital signs, and injections

• Assist in chronic disease support and preventive care initiatives

• Communicate changes in patient status to healthcare providers

• Maintain timely EMR documentation and assist onboarding
Requirements:

• Registered Practical Nurse licensed with the College of Nurses of Ontario

• Preference for two years related clinical experience

• Current CPR certification and professional liability insurance

• Proficient with EMRs and clinical technologies

• Excellent communication and organizational skills
