In this role, you will utilize your expertise in chronic disease management, mental health, and addiction treatment to make a difference. As a registered Nurse Practitioner in good standing with the College of Nurses of Ontario, you will engage actively in patient-centered care while promoting self-care principles. Responsibilities include diagnosing patients, collaborating on care plans, and responding to emergencies.
Key Responsibilities:
• Assess and diagnose patients for health promotion
• Communicate treatment plans clearly to patients
• Collaborate with an inter-professional care team
• Encourage self-care among patients in custody
• Respond to emergency situations with interventions
Requirements:
• Registered Nurse Extended Class with the College of Nurses of Ontario
• Nurse Practitioner specialty certificate in primary or adult care
• Experience in chronic disease management and mental health
• Strong communication and analytical skills
• Commitment to ongoing professional growth
Experience an unconventional setting while supporting the rehabilitation and well-being of individuals in custody at Thunder Bay Jail.
