Our 40-bed skilled nursing center in Fremont, Ohio offers a range of services, including short-term rehabilitation, transitional care, dementia care, and more.

• Provide direct patient care to our residents, including administering medications, monitoring vital signs, and performing treatments as needed
• Develop and implement individualized care plans to meet the unique needs of each resident
• Collaborate with our interdisciplinary team to ensure seamless care coordination and communication
• Monitor and report changes in resident condition to the healthcare team and family members
• Maintain accurate and up-to-date patient records and reports
• Participate in quality improvement initiatives to ensure the highest level of care and service
