This role requires a Master of Nursing – Primary Health Care Nurse Practitioner and four years of nursing experience. Key Responsibilities:

• Manage primary care needs of a patient roster

Requirements:

• Master’s Degree in Nursing – Primary Health Care Nurse Practitioner

• Four years of nursing experience required

• Registered Nurse Extended Class certificate in good standing

• Membership with Nurse Practitioners’ Association of Ontario

• Knowledge of health and social systems essential
