A primary care organization is seeking a dynamic, organized, and reliable Nurse Practitioner to join its interdisciplinary healthcare team. Reporting senior leadership, the successful candidate will collaborate closely with physicians and allied health professionals to enhance access to primary care services for priority populations. The role is performed in accordance with professional standards governing nurse practitioners in Ontario.
Key Responsibilities- Deliver comprehensive primary healthcare services in a community clinic setting
- Travel to partner sites as required
- Support the development and implementation of new community-based healthcare initiatives
- Maintain accurate clinical documentation and patient records
- Contribute to preventive care, risk reduction, and chronic disease management strategies
- University degree as a Nurse Practitioner
- Member in good standing with the College of Nurses of Ontario (Extended Class)
- Experience in a medical clinic setting and familiarity with EMR systems
- Strong communication skills in both official languages
- Excellent organizational skills, professionalism, and ability to work autonomously in an evolving healthcare environment
