The RoleReporting to the Administrative Lead, the Nurse Practitioner will play a key role in delivering comprehensive primary healthcare services within a multidisciplinary team environment. The successful candidate will assess, diagnose, order and interpret diagnostic tests, prescribe medications, and develop treatment plans for patients experiencing episodic illnesses and chronic health conditions.
This role offers the opportunity to provide direct patient care while contributing to health promotion initiatives designed to improve the health and well‑being of individuals, families, and the broader community.
Key Areas of Leadership and OversightPrimary Care Management:
Effectively manage the primary healthcare needs of a roster of patients in collaboration with an interdisciplinary healthcare team.
Clinical Assessment and Treatment:
Assess, diagnose, and treat patients within the Nurse Practitioner scope of practice, including ordering and interpreting diagnostic tests and prescribing medications and therapies.
Health Promotion and Prevention:
Support and lead health promotion initiatives focused on disease prevention, wellness, and healthy lifestyle practices.
Quality and Professional Practice:
Demonstrate and maintain clinical competence in advanced health assessment, diagnosis, therapeutics, and evidence-based patient care.
Collaboration and Care Coordination:
Work closely with healthcare professionals, community partners, and support services to coordinate comprehensive patient care.
The ideal candidate is a compassionate and highly skilled healthcare professional who combines strong clinical expertise with sound judgment, professionalism, and a commitment to patient‑centred care. The successful candidate will possess excellent analytical and critical‑thinking abilities, demonstrate sound decision‑making under pressure, and maintain a strong focus on delivering exceptional healthcare services. They will be an effective communicator and relationship‑builder who values collaboration, continuous learning, and service excellence.
Experience as an educator, preceptor, or involvement in quality improvement initiatives would be considered a strong asset.
Qualifications and Experience- Master of Nursing, Primary Health Care Nurse Practitioner (MN-PHCNP)
- Registration in good standing with the College of Nurses of Ontario as a Nurse Practitioner, Primary Health Care (RN-EC)
- Membership in the Nurse Practitioners' Association of Ontario
- Proof of Professional Liability Protection (PLP)
- Four to five years of nursing experience preferred
- Experience providing primary healthcare services and managing patients with episodic and chronic conditions
- Experience working within interdisciplinary healthcare teams
- Experience using Electronic Medical Records (EMR) systems and related healthcare technologies
- Experience as a clinical educator or preceptor is considered an asset
- Knowledge of quality management practices and continuous improvement principles is considered an asset
- Knowledge of provincial healthcare systems and community-based health and social services
