Nurse Practitioner - East End Family Health

Northern College seeks two Nurse Practitioners committed to delivering high-quality care. You will assess, diagnose, and treat patients, managing episodic and chronic conditions while collaborating within a multidisciplinary team.

Your role will also involve health promotion initiatives to enhance patient well-being and community health.

Key Responsibilities:

• Manage primary healthcare needs of a patient roster

• Diagnose and treat patients according to NP scope

• Order and interpret diagnostic tests and prescribe medications

• Support health promotion and disease prevention initiatives

• Coordinate care with healthcare professionals and community partners

Requirements:

• Master of Nursing, Primary Health Care Nurse Practitioner

• Registered with the College of Nurses of Ontario

• Four to five years of nursing experience preferred

• Experience in Electronic Medical Records (EMR) systems

• Knowledge of health promotion and population health principles
