Nurse Practitioner for Underserved Populations

Be part of TAFHT's mission as a Nurse Practitioner, offering vital healthcare services in Timmins, Ontario. This role focuses on providing primary care to those lacking access and addressing their unique healthcare needs.

We are seeking two compassionate NPs who are committed to working with vulnerable populations. You will be responsible for health assessments, diagnosing, and developing holistic treatment plans. Your collaboration with an interdisciplinary team will create a seamless care experience for patients requiring comprehensive support.

Key Responsibilities:

• Deliver primary care services to unattached patients

• Create and implement patient-centered care plans

• Collaborate with healthcare and community professionals

• Participate in health promotion activities

• Monitor patient progress and adjust care plans

Requirements:

• Must hold a valid NP registration in Ontario

• At least 2 years of independent practice experience

• Proficient in electronic medical records

• Excellent clinical and communication skills

• Commitment to community health advocacy
