Nurse Practitioner - Diabetes Care Specialist

As part of the Diabetes Program, you will work closely with interdisciplinary team members to deliver comprehensive care. This role requires advanced assessment skills, effective treatment prescribing, and a commitment to patient education. You will be instrumental in fostering an environment where patients feel empowered to manage their diabetes effectively.
Key Responsibilities:
• Conduct and document thorough client assessments
• Develop and communicate care plans with the healthcare team
• Identify health education needs and provide guidance
• Order and interpret diagnostic tests as needed
• Facilitate referrals to specialists and health services
Requirements:
• Must be a registered Nurse with Ontario's College of Nurses
• 2+ years of experience with complex medical issues
• Previous NP experience in outpatient settings is an asset
• Proven ability to work independently
• Bilingual English and French proficiency required
