Reporting to the Ontario Works Program Manager, the Case Manager will engage with clients to assess needs and co-create effective plans. You will play an integral part in fostering community connections, mentoring clients, and navigating barriers to success. This role involves meticulous case management to ensure clients receive the needed support from various programs.
Key Responsibilities:
• Co-create client goals for pathways to success
• Provide comprehensive case management support
• Engage clients in necessary application processes
• Liaise with service providers to facilitate support
• Monitor case files for compliance and progress
Requirements:
• In-depth understanding of community service programs
• University degree or college diploma in Social Sciences
• Minimum of two years related experience
• Strong analysis and reasoning skills
• Valid driver’s license and ability to work out of office
