- A Registered Nurse, Shore Care at Hackensack Meridian Health involves the delivery of nursing practices according to the most up-to-date evidence-based research, nursing policies, and standards of care, to provide and ensure safe and excellent delivery of individualized patient care.
- Day in the life includes assessment, planning, implementation, and evaluation.
- Collects comprehensive data pertinent to the patient's health or the situation.
- Performs age and disease specific assessment/data collection independently in a systematic manner.
- Recognizes clinical and diagnostic status changes in patient status.
- Identifies variables in patient/family education needs.
- Develops a plan of care that prescribes strategies and interventions to attain expected outcomes.
- Formulates age and disease specific patient/family centered goals/outcomes based on assessment data.
- Communicates, coordinates, and implements the plan of care.
- Provides ongoing communication with patient/family, nursing colleagues and other health care team members.
- Implements age and disease specific care in an organized and timely manner.
- Provides or coordinates the patient/family education as needed and ensures understanding.
- Performs according to established protocols in clinical situations, including communication with appropriate persons and documenting events.
- Provides nursing care and treatments that reflect the patients' needs, advocates appropriately and demonstrates evidence-based nursing practice and compliance with standards.
- Performs continuous point-of-care electronic documentation of each patient's assessments, plan-of-care, interventions and evaluation of rendered care.
- Medication administration utilizing the latest technological advancements.
- Participates in multidisciplinary discharge planning to ensure safe transition.
- Evaluates progress toward desired outcomes and initiates interventions as needed.
- Identifies ethical issues and brings them to the attention of team members.
- Recognizes opportunities to enhance patient satisfaction.
- Acquires and maintains required competencies, certifications and continuing education.
- Adheres to the American Nurses Association Code and HMH Organizational standards.
- Willing to travel between all HMH locations.
- Long periods of sitting with frequent standing, walking or moving.
- Requires periods of keying on computer terminal to input information.
- Requires constant verbal communication in person or by phone.
- Requires frequent reading of correspondence, email, reports, material, etc.
- Exposure to communicable diseases.
- Lifting >35 pounds.
- Extended wearing of PPE.
- Application of surgical mask for long periods of time.
- Lifting a minimum of 35 lbs., pushing and pulling a minimum of 10 lbs. and standing a minimum of 6 hours a day.
Skills and Abilities
Required
- Associate's degree in Nursing or Diploma in Nursing.
- Minimum of two (2) years recent experience in specialty (effective May 5, 2025).
- Minimum of one (1) year experience in a floating professional environment (effective May 5, 2025).
- Ability to learn quickly and adapt to changing patient needs.
- Strong sense of accountability for improving the lives of our patients and their caregivers.
- Exceptional focus on teamwork, dedication to ongoing education, and passion to deliver highest quality care.
Skills And Abilities
Preferred
- National Certification in area of specialty preferred when eligible.
- Bachelor's degree in Nursing.
