Registered Nurse - Shore Care - Telemetry - PT Days with Benefits
Registered Nurse role in Shore Care Telemetry at Hackensack Meridian Health. The RN manages complex telemetry patients, ensuring outcomes, coordinating across interdisciplinary teams, and providing direct, evidence‑based nursing care.
OverviewAt Hackensack Meridian Health, the RN provides direct patient care for telemetry patients, focusing on advanced assessment, care planning, and collaboration across the health care team to achieve optimal patient outcomes.
Responsibilities- Collect comprehensive data pertinent to the patient's health or the situation.
- Perform age and disease‑specific assessment and data collection independently in a systematic manner focusing on physiologic, psychological, and cognitive status.
- Recognize clinical and diagnostic status changes; identify variables in patient/family education needs based on age, disease, and culturally specific learning differences.
- Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
- Formulate age and disease‑specific patient/family centered goals/outcomes based on assessment data.
- Develop and coordinate a plan of care that is patient/family focused, prioritized and encompasses the continuum of care.
- Communicate, coordinate, and implement the plan of care.
- Provide ongoing communication with patient/family, nursing colleagues, and other members of the health care team regarding patient status, rendered care, and treatment plans.
- Implement age and disease‑specific care in an organized and timely manner.
- Provide or coordinate patient/family education as needed and ensure understanding.
- Perform according to established protocols in clinical situations, including communicating with appropriate persons and documenting events.
- Provide nursing care and treatments that reflect patient needs, advocate appropriately, and demonstrate evidence‑based nursing practice and compliance with standards.
- Perform continuous point‑of‑care electronic documentation of each individual patient's assessments, plan‑of‑care, interventions, and evaluation of rendered care.
- Administer medication utilizing the newest advancements in bar‑scanning and documentation technologies designed to ensure the safest delivery of medications to our patients.
- Discharge patients in a multidisciplinary manner to ensure safe transitioning and continuum of care.
- Evaluate progress toward attainment of outcomes.
- Utilize critical thinking skills based on nursing knowledge to achieve desired patient outcomes.
- Initiate interventions related to patients' change in condition and unexpected response to care.
- Identify ethical issues in the practice setting and bring them to the attention of other team members.
- Recognize and respond to opportunities to enhance patient satisfaction.
- Acquire and maintain competencies, certifications, and continuing education to meet facility accreditation or individual licensing requirements.
- Adhere to the American Nurses Association Code for Nurses with Interpretive Statements.
- Travel between all HMH locations as required.
- Lift a minimum of 35 lbs.; push and pull a minimum of 10 lbs.; stand a minimum of 6 hours a day.
- Adhere to HMH's Organizational and Managerial competencies and standards of behavior.
- Perform other duties and/or projects as assigned.
- Long periods of sitting with frequent standing, walking, or moving.
- Require periods of keying on a computer terminal to input information.
- Require constant verbal communication in person or using the phone.
- Require frequent reading of correspondence, email, reports, and material.
- Exposure to communicable diseases.
- Lifting > 35 pounds.
- Extended wearing of PPE.
- Application of surgical mask for long periods of time.
Education,
Knowledge, Skills and Abilities Required:
- Associate's degree in Nursing or Diploma in Nursing.
- Minimum of two (2) years recent experience in the required specialty.
- Minimum of one (1) year experience in a floating professional environment such as a travel professional organization, site‑flexible resource float pool, or equivalent.
