Description

Under the general direction of the Director of Clinical Operation, the Clinical Manager is the leader of the Patient Care Team and is responsible for supervising, evaluating and coordinating the various members of the Interdisciplinary Team. The role directs patient care services for the team and serves as patient advocate and coordinator for health care providers in the community who are involved in the care of the team’s patients.

Position is salaried, full time, exempt.

Education & Training Required

Associate’s Degree in Nursing;
Bachelor’s Degree in Nursing preferred. One year of supervisory experience in a hospice or similar position preferred; or two years of successful supervisory experience or equivalent in a health care organization.

Certifications / Licenses Required

Current and valid Florida RN license. Certification in Hospice and Palliative Nursing required within one (1) year of hire.

Experience / Specific

Skills and Abilities

Required

Possess and utilize excellent English oral and written communication skills. Proficient in Microsoft Office Software and preferably knowledgeable in using a hospice clinical system (e.g., Misys).

Physical Requirements

Ability to lift, push, pull twenty‑five (25) pounds without difficulty; good visual and audio acuity; able to work under varying degrees of stress; ability to work in various facility and home environments, including those with exposure to tobacco, smoke, and pets.

Primary Responsibilities
  • Direct patient care services for the team.
  • Review information on all pending patient admissions, new admissions and “live” discharges.
  • Maintain complete and accurate data and documentation in the medical record.
  • Monitor the type and level of team services provided (staffing, medications, supplies, level of care changes) to ensure appropriateness and responsiveness to patient/family needs.
  • Manage team schedule and coordinate services.
  • Facilitate IDT meetings, ensuring interdisciplinary input into the plan of care.
  • During periods of low census or staff shortages, fill in and provide patient care within scope of practice.
  • Participate in clinical on‑call rotation.
  • Perform other duties as assigned.
Staff Supervision and Management
  • Interview, select, train, supervise, evaluate, and recommend dismissal of team staff in collaboration with other management staff.
  • Ensure staff competence and performance through field visits, documentation evaluation and providing team‑based educational in‑services.
  • Act as resource and mentor for staff regarding clinical issues, documentation, inter‑ and intra-team problem solving, and appropriate customer service behavior.
  • Review and approve payroll, ensuring documented accurate time, mileage, and expenses.
  • Ensure that licensed practical nurses, hospice aides, and other care assistants work under RN supervision in conjunction with the Director of Clinical Operations.
