We are looking for an enthusiastic and motivated Community Occupational Therapist to join our Community Health and Social Care Team on a 12-month fixed-term contract. This is an excellent opportunity for a qualified Occupational Therapist who is passionate about supporting adults to remain as independent as possible within their own homes and communities.

Working as part of a multidisciplinary team, you will undertake strengths-based assessments, identify practical solutions to promote independence, and support individuals to achieve outcomes that matter to them. You will work with adults with a wide range of physical disabilities, health conditions and support needs, providing specialist Occupational Therapy assessment, advice, equipment recommendations, moving and handling solutions, and adaptation recommendations.

About the Role:

You will:

• Complete person-centred Occupational Therapy assessments.
• Promote independence through a strengths-based approach.
• Assess for and recommend equipment, minor and major adaptations.
• Undertake moving and handling assessments.
• Work collaboratively with colleagues across Health, Social Care, Housing and voluntary sector organisations.
• Contribute to safeguarding responsibilities and Care Act assessments where appropriate.
• Manage a varied caseload with support and supervision from experienced Occupational Therapy colleagues.

We are looking for someone who:

• Holds a Degree or Diploma in Occupational Therapy.
• Is registered with the Health and Care Professions Council (HCPC).
• Has excellent assessment, communication and problem-solving skills.
• Is committed to person-centred and strengths-based practice.
• Works effectively as part of a multidisciplinary team.
• Is able to prioritise workload and manage competing demands.
