Job Overview/Summary

We are currently seeking a dedicated Abdominal Transplant Clinical Pharmacist Specialist to join a high-performing transplant care team. This role focuses on delivering expert pharmaceutical care to abdominal transplant patients, ensuring optimal medication management, patient safety, and therapeutic outcomes. The specialist will collaborate closely with physicians, surgeons, and other healthcare professionals to support clinical decision-making and pharmacy operations within the abdominal transplant service line.



Key Responsibilities

  • Participate in specialized medication management for abdominal transplant patients, ensuring safe and effective use of immunosuppressants and other therapies.
  • Engage in clinical programs to improve patient outcomes, optimize pharmacotherapy, and promote wellness and disease prevention.
  • Perform comprehensive medication reviews and medication therapy management (MTM) to achieve targeted therapeutic goals.
  • Support medication distribution systems and operational functions to ensure timely availability and accurate processing of prescriptions.
  • Promote and lead cost-containment initiatives regarding drug utilization and pharmacy resource allocation.
  • Collaborate with the interdisciplinary transplant team to contribute to treatment planning, medication selection, and care coordination.
  • Identify and resolve potential or actual adverse drug events (ADEs), including reporting and trending for quality improvement.
  • Educate patients and healthcare team members on medication use, compliance, and safety strategies.
  • Serve as a preceptor to pharmacy students and residents and contribute to internal staff training and education initiatives.
Primary Skills

  • In-depth knowledge of abdominal transplant pharmacotherapy and immunosuppressive drug regimens
  • Advanced medication therapy management (MTM) and clinical decision support
  • Strong communication and collaboration with multidisciplinary healthcare teams
  • Medication safety evaluation and adverse event reporting
  • Operational and order-processing proficiency in pharmacy systems
Secondary Skills (Good To Have)

  • Experience with transplant-specific protocols and multidisciplinary transplant boards
  • Preceptor or academic training experience with students or residents
  • Proficiency in using pharmacy informatics and EMR systems (e.g., Epic, Cerner)
Qualifications

  • Graduate of an ACPE-accredited School of Pharmacy with a BSPharm or Pharm

    D (required)
  • Advanced pharmacy training or clinical residency (PGY1 or PGY2 in transplant or critical care) (preferred)
  • Active pharmacist license in the practicing state (required)
  • Basic Life Support (BLS) certification (preferred)
