The Social Worker - MSW is responsible for the planning, coordination, and delivery of quality medical social work services in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
Essential Activities and Tasks Care and Expectations Management - 45%- Provides ongoing assessment of patients, assuring an accurate evaluation of patient/family needs. Develops a comprehensive care plan that addresses the psychosocial needs of the patient and family. Makes home health visits as needed.
- Works with patients and families regarding coping with illness, accessing resources, or other issues affecting the patient’s health status. Links patient/family with needed services. Assists patients in arranging for services and funeral planning.
- Must supervise and cosign all care plans created by Licensed Social Workers (LSWs).
- Plans for and participates in Interdisciplinary Team meetings.
- Collaborates with Interdisciplinary Team members regarding crisis situations and on-going care of patient/family.
- Evaluates bereavement risk at the start of care, with continued evaluation of bereavement needs.
- Follows current social work professional standards of practice and agency approved protocols and monitors all psychosocial-care plans and makes changes to care plans as needed.
- Maintains current and accurate patient records thorough use of laptop or desktop computers, electronic medical record, and paper/chart documentation in accordance with company policies, federal, state, and local guidelines.
- Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers.
- Participates in quality improvement projects as needed.
- Develops and maintains close working relationship with community resources and referral sources. Markets social work/care management services to achieve volume goals.
- Provides direction in the orientation, in-service and continuing education of home health care staff as requested; keeping the staff current with changes in health care procedures and policies as needed.
- Commitment to quality customer service for internal and external customers.
All other duties as assigned.
Qualifications Education- Master’s degree in social work accredited by the Council on Social Work Education required.
- Current unencumbered license for the state of Ohio to practice as a Licensed Social Worker (LSW) required.
- One year experience in social work in a health care setting required.
- Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
- Must be able to read, write, speak, and understand the English language.
- Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.

- May be exposed to infectious waste, disease, conditions, etc. including exposure to the AIDS and Hepatitis B viruses - Work day
