Position: Care Coordinator - Huron County

Job Summary

Harbor is seeking a Care Coordinator who is responsible for comprehensive, multi‑system care coordination activities for youth with complex needs and their families. This full‑time role (40 hours per week) is remote and in the community; candidates who live in Huron County are preferred.

Responsibilities
  • Effectively engage children and youth with significant behavioral health needs and their family/caregivers for assigned caseloads (1:10 for ICC and 1:25 for MCC).
  • Build and maintain knowledge of available community resources and help link youth and families to needed supports.
  • Conduct comprehensive CANS assessments and develop individualized, strengths‑based Child and Family‑Centered Care Plans.
  • Actively monitor plans to ensure services are provided, address challenges and barriers, and update or modify plans as needed.
  • Facilitate Child and Family Team meetings, ensuring children and families have a voice and choices in their plans and provider selections, encouraging creative solutions and natural supports to maintain youth in the community.
  • Facilitate access to clinical and psychiatric consultation for assigned ICC and/or MCC youth.
  • Develop safety‑crisis plans with youth and families and review them.
  • Ensure initial and ongoing engagement, contacts, and documentation are completed as required per CME rules.
  • Be willing to work flexible hours, including evenings and/or weekends.
  • Provide services in the home and other community settings.
Qualifications and Experience
  • At least 3 years of experience providing community‑based services to children, youth, and families or caregivers in behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or related public‑sector human services or behavioral health fields (or 2 years with an associate or bachelor’s degree; or 1 year with a master’s degree).
  • Background and experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
  • Culturally competent or responsive with training and experience necessary to manage complex cases.
  • Experience working with children and families experiencing SED, trauma, co‑occurring behavioral health disorders and who are engaged with one or more child‑service systems.
  • Completion of the high‑fidelity wraparound training program provided by an independent validation entity recognized by the ODM (the COE).
  • Excellent verbal and written communication skills and willingness to represent Harbor within the medical/health‑care community.
  • Proficient and accurate computer use.
  • Valid driver’s license, acceptable driving record, and insurability through Harbor’s vehicle insurance carrier.
  • CPR/First Aid and NCI/CPI certifications required within 90 days of employment.
  • Preference for staff who reside in the community they serve.
  • Minimum Internet service requirements: minimum upload 5 mbps; minimum download 10 mbps per user; provider must be cable or fiber (DSL, satellite, cellular not permitted); business class preferred. Ability to connect Harbor computer directly to modem/router with an ethernet cable (not wireless).
