We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our client’s expanding team in Toledo, Ohio. This position focuses on delivering high-quality, patient-centered care in a home setting, helping individuals achieve their health goals and maintain independence. The ideal candidate will bring strong clinical expertise, excellent communication skills, and a commitment to providing exceptional patient care.

• Conduct comprehensive patient assessments and provide skilled nursing care in home environments
• Develop, implement, and evaluate individualized care plans in coordination with physicians and other healthcare professionals
• Educate patients and families on medical conditions, medication management, and self-care practices
• Monitor patient progress, document clinical observations accurately, and adjust care plans as needed
• Ensure compliance with all regulatory and documentation standards
• Collaborate with the interdisciplinary team to promote optimal patient outcomes and continuity of care

• Monday through Friday, 8:00am–5:00pm
• One weekend per month (on-call)
• Two holidays per year
• Flexible scheduling with minimal weekend work
• This is a Pay Per Visit position

• Local travel required throughout the Toledo area (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Ohio
• Minimum of 1 year of professional RN experience (home health preferred)
• Current BLS or ACLS certification through the American Heart Association
• Valid driver’s license, reliable transportation, and current auto insurance

• Prior experience in home health or community-based care
• Strong organizational, communication, and assessment skills
